Our service coordinators help people explore what they want and need in life, and then assist them in reaching their goals. Those eligible include: Medicaid, Department of Health or Nursing Home Diversion Waiver If you need professional assistance in putting a support plan together our staff of service coordinators can help you. They work in partnership with the person or family to develop, implement, and maintain the person’s life plan. This life plan describes who the person is, his or her strengths, capacities, needs, valued outcomes, and the supports and services he or she wants or needs to achieve these outcomes.
Service Coordination

Service coordinators help people attain the highest quality of life and live as independently and productively as possible.
This includes:
Obtain and coordinate supporting information for planning process
Help to identify and achieve valued outcomes or personal goals
Assist the individual in the coordination of his/her individualized person centered plan; assuring that all referrals and associated linkages needed to achieve the valued outcomes are made with the individual
To oversee the agency and program supports of the individual ensuring that the person’s interests, preferences, and capabilities along with inclusion into the community, are maintained
Promote self-advocacy and choice
