Our Service Coordinators help people explore what they want and need in life,   and then assist them in reaching their goals.  Those eligible include: Medicaid,  Department of Health or Nursing Home Diversion Waiver. If you need professional assistance in putting a support plan together our staff of Service Coordinators can help you.  They work in partnership with the person or family to develop, implement, and maintain the person’s life plan.  This life plan describes who the person is, his or her strengths, capacities, needs, valued outcomes, and the supports and services he or she wants or needs to achieve these outcomes.
