Service Coordinators help 605 people attain the highest quality of life and live as independently and productively as possible. They work in partnership with the person and/or family members to develop, implement and maintain an individual’s life plan. This life plan describes who the person is and his or her strengths, capacities, needs, valued outcomes and the supports and services he or she wants or needs to achieve these outcomes.The Service Coordinator is at the core of the advocacy and supports for each individual. Lexington’s Service Coordination services for people with developmental disabilities have a 5-star rating from the Office for Persons with Developmental Disabilities.
